Agreement to Terms
These Terms and Conditions ("Terms") govern your use of Mood & Mind’s website, portals, and services (collectively, the "Services"). By accessing or using the Services, you agree to be bound by these Terms and our Privacy Policy/Notice of Privacy Practices. If you do not agree, do not use the Services.
Not Medical Advice & Emergencies
Content on our website and any general communications are for informational purposes only and are not a substitute for professional diagnosis or treatment. If you are in crisis or considering harming yourself or others, call 988 (Suicide & Crisis Lifeline), 911, or go to the nearest emergency room. Do not rely on electronic communications for emergencies.
Eligibility & Patient Relationship
A patient‑provider relationship is formed only after you complete intake procedures, we accept you as a patient, and you receive confirmation of scheduled care. We may decline or discontinue services consistent with law and ethical guidelines (e.g., scope of practice, safety concerns, non‑payment, inappropriate conduct).
Insurance, Billing & Financial Responsibility
We currently accept the following plans: Aetna, Blue Cross Blue Shield, HealthPartners, Medicare, Medicaid, MNSure, UCare (participation and coverage vary by plan). It is your responsibility to verify our participation with your specific plan and to understand benefits, deductibles, co‑pays, co‑insurance, prior authorization needs, and any coverage limitations.
Payment: Co‑pays and patient‑responsible amounts are due at the time of service.
Claims: We may submit claims to your insurer; any unpaid balances are your responsibility.

Patient Responsibilities
Provide accurate information; participate in treatment; follow safety recommendations; use medications as prescribed; keep appointments; and communicate changes in symptoms, medications, or insurance.
Communications & Portals
Non‑urgent messages may be sent through our patient portal or other approved channels. We attempt to respond within [X] business days. Electronic messages are not continuously monitored and are not appropriate for emergencies.
Website Content & Intellectual Property
All website content (text, graphics, logos, design) is owned by Mood & Mind or licensed to us and protected by intellectual property laws. You may not copy, distribute, or create derivative works without prior written permission.

In no event will our total liability exceed the amount you paid to us for the applicable service in the 3 months preceding the claim.
Indemnification
You agree to indemnify and hold harmless Mood & Mind and its personnel from claims, damages, losses, and expenses (including reasonable attorneys’ fees) arising from your violation of these Terms or misuse of the Services.
Nondiscrimination
Mood & Mind complies with applicable federal and Minnesota civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, sex, gender identity, sexual orientation, or any other status protected by law. Governing Law & Venue
These Terms are governed by the laws of the State of Minnesota, without regard to its conflict‑of‑laws rules. Exclusive venue for disputes lies in state or federal courts located in Olmsted County, Minnesota, unless otherwise required by law.
